New Patients

Please call (815) 223-0171 to schedule your new patient exam. Every new patient must fill out all required documents prior to appointment. We can send the paperwork ahead of time or you can print them and bring them to your appointment. The required forms are located under “Patient Resources” on the Welcome Page of our website. Please take a few moments to fill out and sign all forms. This will help us expedite your initial visit at our office.

First Appointment

Dr. Ludford feels that the first appointment is the time he will spend getting to know you and your dental needs. He will do an overall evaluation of your gums and teeth as well as addressing any health problems that may be of concern. His assistant will take the recommended full series of x-rays that he will need to evaluate your dental needs. Dr. will then discuss his findings and discuss the options and recommendations he has. With a complete evaluation of the mouth, Dr. is able to design a personalized treatment plan. From your treatment plan our office manager is able to pretreat for any major work with your insurance company.

Financial

We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. For patients who require major work, a complete payment plan is designed with an appropriate payment schedule. Forms of payment accepted by the office are check, cash, Visa, MasterCard, or Care Credit.

Cancellation

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours’ notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
